• Submission Method: Applicants are requested to complete the relevant forms and arrange all required documents in the specified order. 0-2. Faculty Curriculum Vitae (CV)

There is no prescribed format. The CV must include the following supporting documents:

• Copy of the Teacher Certificate.
• Copy of the Highest Academic Degree Certificate.(For degrees obtained overseas, the certificate must be authenticated by a Taiwan representative office abroad.)
• Copy of the Medical License.
• Copy of the Specialist Physician Certificate.
• Documentation of Relevant Professional Experience.
• List of Publications Published or Accepted for Publication within the Past Five Years.(For applicants for the positions of Associate Professor or Assistant Professor, the title of the doctoral dissertation must also be included.)Publications must be categorized, such as journal articles, conference proceedings, and books/monographs. Publications within the past five years indexed in SCIE, SSCI, AHCI, TSSCI, or THCI (Core) should be clearly indicated. Please also indicate whether the applicant is the first author or corresponding author.
• Copy of Patent Certificates, together with a description of the patent's contribution to industrial technological development and its specific impact and outcomes. Not required for applicants with no patent records.
• Copy of Technology Transfer or Copyright Licensing Agreements, together with a description of how the technology transfer or licensing has contributed to industrial technological development or had an impact on the country or society, including specific outcomes. Not required for applicants with no technology transfer or licensing records.
• Copy of Award Certificates for Inventions, Innovations, or Technology Transfer. Not required for applicants with no relevant awards.
• List of National Science and Technology Council (NSTC) Industry-Academia Collaboration Research Projects and Approved Funding. Not required if none.

• List of NSTC Research Projects and Approved Funding. Not required if none.
